This Vet Credit is good for one year from the date of puppy purchase.

THIS IS A ONE TIME VET CREDIT DISCOUNT OF $__________________ from the total puppy purchase price to use for vet visits, vaccines or any prescription or non prescription medication that the puppy may need or any supplies needed for puppy.

This savings could be used for hernia repair, de-wormer, stool sample, blood test, x-rays, Advantage or Front Line Flea Treatment, ear mite medicine, rabies vaccine, or whatever pet charges your puppy incurs. No other medication or vet refunds will be issued for this puppy purchase. However, if the puppy has a health problem that cannot be treated with minor medication, will require long term medical expenses, or major surgery, it will be replaced in accordance with the health guarantee received at time of purchase.

This Vet Credit Discount overrides Original Health Guarantee.

By purchasing the puppy at a discounted price, I understand I am not receiving a Lifetime Health Guarantee on this puppy.
The Lifetime Health Guarantee is available only when a puppy is purchased at the original starting price.

Remember, there is pet insurance on the market today that can be purchased for a small fee. We recommend that all our customers purchase some type of pet insurance to help with the cost of vet visits or emergency vet services. Ask your vet about purchasing pet insurance.
