We now offer you the choice to have a " Detailed Pet Health Exam" done before you get your puppy. The cost is $200.00
We will bill your credit card for the charges or you can include it in your bank transfer if you have not already submitted your payment.

Regardless of your choice, you must sign and return this form before your puppy can be shipped.
If you have selected option # 2 your puppy will be shipped within 10 days.

Shipping Customers: Print this form, fill in the blanks, sign and send by fax or scan it in and send it to us by e-mail. Must be returned before we can ship your puppy!

In-Store Customers: Must be completed prior to picking up puppy in person.

We now offer you the choice to have a " Detailed Pet Health Exam" done before you get your puppy. The cost is $200.00

At the time of the health exam we will ask for a licensed veterinarian to check for the following items listed below. We will give you a full report along with his/her contact information.

1. Dental Exam: Correct bite, no missing teeth, no abscesses, and etc.
2. Ear Exam: Ears are free from mites, inflammation, and visible signs of infection, etc.
3. Eye Exam: Eyes are free from entropia, evidence of corrective surgery, corneal scars, cataracts and visible flaws.
4. Fecal Exam: Free of any parasites, coccidian, Giardia and other GI problems.
5. Glucose, Anemia: Free of glucose or anemia problems.
6. Hair and Skin Exam: Free of mange, fleas, ticks, lice, other parasites, infections and any abnormal skin conditions.
7. Heart Exam: Free of heart murmur.
8. Hernia Exam: Free of hernia
9. Open Fontanella:
10. Leg Exam: Visually Noticeable Luxating Patella, other MS problems
11. Reproductive Exam: Male has two testicles.
12. Respiratory Exam: Lungs have a normal respiratory sound on both sides. Free from congestion and cold symptoms. 

 A stool sample is not taken on puppies prior to selling or shipping puppies and is not a part of the health certificate that is required for shipping your puppy. It is required that a stool sample of the puppy you purchased be done by a licensed veterinarian within 2 to 3 days (depending on the size of puppy) from the date your puppy is received, in order for the purchase agreement to be valid.
If you choose to have the Health Exam done prior to shipping, we will notify you of the results as soon as we have them. It may slightly delay the shipping of your pet as we discuss the results and shipping options for your puppy. Yes, you will need to have another stool sample and exam done, once the puppy arrives at your location within the time allowed. Many of our customers prefer to save on the cost of their new puppy and only have this done once at their location. That is why we offer it as an option.
The reason we require you to have an health exam and stool sample done on your puppy when it arrives is because it has been exposed to other pets while in transit to it's new home.

Regardless of your choice, you must sign and return this form before your puppy can be shipped or sold.
If you have selected option # 2 your puppy will be shipped within 10 days.
