A donation application form must be submitted if you are requesting any type of donation, gift certificates or a donation puppy from our pet boutique.
There is a $25.00 non refundable application fee that must be paid before your donation request will be accepted.
All applications submitted are guaranteed a donation from our pet boutique, if we are able to verify that you are a non profit organazation, church or part of a public/ private school.
