CREDIT CARD SIGNATURE VERIFICATION FORM # 3
Due to the recent increase in credit card fraud we now need to verify each person purchasing a pet from us.
This is to protect us as well as you the customer.
The name on the credit card or check must match the signature of purchaser. No Exceptions!
When we have verified that the name and signature match the signature on the credit card we will shred and destroy your this form 10 days after your puppy has been shipped.
Once we receive your authorization we will escalate your Puppy Purchase to the next level.
If your puppy requires shipping, it will be processed for the next scheduled shipping day.
State Sales Tax in the amount of 8.25% is applied when shipping your puppy anywhere in Texas.
2. Enter payment information with signature.
REMINDER: The person that makes payment must be the same as the person that will pick the puppy up at the airport.
Signature of person authorizing payment will be required at the airport along with valid I.D.
