INSTRUCTIONS:

MANUAL CREDIT CARD AUTHORIZATION FORM # 1
This form authorizes charges for puppy purchase only, shipping will be charged seperately.

1. Click Open & Print Puppy Purchase Authorization Form # 1 Below.
2. Enter payment information with signature.
The person that makes payment must be the same as the person that will pick the puppy up at the airport.
Sorry no exceptions, if you need someone else to pick your puppy up at the airport you will need to do a bank transfer that is the only way another person can be added as an authorized person that pet can be released to.

Important! Signature of person authorizing payment will be required at the airport along with valid I.D.

(This is an internet fax by e-mail service and is limited to receive only 2 pages at a time, if you include a cover sheet send only one page at a time.)

If payment is not received 24 hours after puppy purchase application is submitted hold may be released if another application comes in for the puppy you are requesting to purchase.

Return credit card authorization form as soon as possible if you are in a hurry to receive your puppy or do not want to risk loosing the puppy to another applicant.

We never want to make you feel rushed, however if other applications have came in for this puppy, it may be sold to the first person that confirms they are ready to proceed with payment.

You may submit it now if you are ready to proceed with payment or contact me when we open if you have questions that need to be answered before returning this form.

We may have other applicants eager to purchase the puppy and don't want to pass up the opportunity to find a home for this adorable little puppy.
