By signing this agreement, I state that I am the person authorized to make purchases with this credit card. I agree to pay the total amount shown above in compliance with the card holder’s agreement. I also understand that my puppy will not be shipped or cannot be picked up until Teacup and Toy Pets has received this form with my signature and a copy of my driver license and credit card.
