Submit this adoption application only if you would like to adopt a specific puppy that is available for sale with a puppy number on our web site.

 Important Reminders

Once you have submitted your application you will be required to confirm that you are ready
to proceed with your puppy adoption by phone and e-mail as instructed on application confirmation page. 

You will receive a confirmation from us by phone or e-mail letting you know that the pet is available for you to purchase. 

If you have not confirmed you are ready to proceed please do so as soon as possible, to avoid the pet in question being sold to another applicant that may have submitted an application on the same puppy or pet.

Coupons cannot be used on adoption pets.

Local Customers: You will be given 4 business hours on weekdays from the time you submit your adoption application to pick the puppy up in person.
Due to the large amount of people that come into our location on Saturday looking for adoption pets we only allow 2 hours for you to pick your puppy up in person on Saturday.

Customers that need shipping: If pet shipping is allowed for the adoption pet payment must be received within 4 hours of the time we have made the puppy available to you.

The primary e-mail address that you enter on this form is were we will send all of the documents and confirmations that are associated with your puppy purchase. You must be able to download them and print them out from your computer.
Pet purchase forms that we send you must all be signed and returned by fax or scanned and sent back to us in an e-mail 72 hours prior to shipping your puppy.

Instructions for Pet Purchase

For more information on purchasing one of our puppies, visit the information section of our web site.

The reason all puppies are sold as is, if the adoption agreement is not signed: Pets will be sold as is if adoption agreement with care instructions were not read and signed at time of purchase.

We will occasionally have a customer that will not take the time to read and return the health guarantee with important puppy care instructions for their new puppy, and when a problems occurs that could have been avoided if they would have taken the time to read it. They become upset and fell that we should be responsible for paying their vet bills or replacing the pet.

 Their are many people that have never owned a pet and do not understand the care needed to raise a small puppy.

You will have the opportunity to read the adoption agreement, size and health guarantee when you come into our walk in location. Or you may return it to us by certified mail within 5 days of the time you receive your new puppy is shipping is needed.

Basically what we are trying to help our customers see is that we want our puppies to go to good homes where they will be loved and appreciated.

If you are willing to take the time to read the care and training instructions for your new puppy, we feel that you will be able to provide the proper care needed for your new puppy.
Therefore we are willing to offer you the full terms of our health guarantee if signed and returned by certified mail.

We realize you don't want to purchase a puppy from anyone that will not stand behind the puppy you have purchased and provide you with a health guarantee.
Therefore we feel that you will understand that we do not like to sell our puppies to anyone that is not willing to take the time to learn about the care needed for new puppies.

 EXAMPLE OF PROBLEMS WITH PREVIOUS CUSTOMERS THAT DID NOT READ THE
HEALTH GUARANTEE & CARE INSTRUCTIONS.
1. If you do not complete your puppies vaccines if needed it could contract a contagious disease and become terminally ill.
 2. We also fell that it is mandatory that you learn and read about hypoglycemia, if your puppy does not eat within the first 4 hours you receive it, the puppy could become lethargic and have a seizure.
3. If you do not take your puppy in for a fecal and it contracted a parasite it could become fatal.
